Introduction to Breast Augmentation

Breast augmentation is frequently sought out in response to natural changes that influence the size and shape of the breasts. Breastfeeding, menopause, and weight loss, for example, can all lead to a decrease in breast volume. When breasts lose their original fullness, the presence of excess skin can make them appear flat and less youthful. Breast implants are an excellent solution for this issue because they support the patient’s existing breast tissue and create pert, natural-looking contours. Breast implants are also frequently used to augment small breasts and give the patient’s figure a classic “hourglass” shape.

Breast augmentation has evolved a great deal since it was initially developed. Today, implants made from biocompatible silicone offer patients a safe, durable option for adding volume to their breasts. Silicone implants are generally considered superior to saline implants because they more accurately mimic the look and feel of natural breast tissue.

Choosing an Implant Type

Dr. Wilson utilizes two different types of silicone implants: traditional silicone implants and form-stable “gummy bear” implants. Traditional silicone implants are filled with a viscous silicone gel, whereas gummy bear implants are filled with a highly cohesive form of silicone gel that firmly maintains its shape. Like a gummy bear candy, they maintain their shape perfectly even when they’re cut in half. Gummy bear implants are available in both shaped and round forms. Teardrop shaped implants may more closely mimic the natural slope of the breast.

Both of these implant types have distinct advantages: Gummy bear implants are favored by some patients because they’re excellent at retaining their shape over the course of many years; they’re also highly resistant to rippling and leakage. Traditional silicone gel implants, on the other hand have a softer feel that’s reminiscent of natural breast tissue. What Happens During Breast Augmentation?

Before you have surgery, Dr. Wilson will provide you with everything you’ll need to recover comfortably. You’ll be given a prescription for pain medication and a special compression garment to wear after your operation. You will need to wear this garment in place of a conventional bra in order to minimize swelling and speed healing.

Breast augmentation is performed while the patient is under general anesthesia, so you’ll be asleep during your operation. This approach ensures complete patient comfort. Once you have been sedated, Dr. Wilson will make a small incision in each breast, inside the inframammary fold. Placing incisions in the fold below the breast effectively hides any scarring beneath the natural overhang of the breast. Inframammary incisions are also necessary for the placement of gummy bear implants. (These implants cannot be compressed for insertion through a more distant incision.)

Usually, breast implants are placed in a pocket directly below the pectoralis muscle. This placement technique uses the patient’s muscle tissue to disguise the upper portion of the implant. This preserves the natural slope of the breasts and stops them from taking on an overly round look. However, subglandular placement (where implants are inserted above the pectoralis muscle) is sometimes a viable alternative for patients who have a substantial amount of natural breast tissue.

Breast Augmentation Recovery

After you wake up from your surgery, you will feel tired and groggy due to the lingering effects of anesthesia. As such, you will need to have someone drive you home from the hospital. You should also ask someone to act as your caregiver for approximately two days after your operation. You may find performing normal everyday tasks difficult during this period due to soreness and swelling.

Most patients report that they feel much better approximately two days after their operation and almost completely recovered at one week. At this time, they’re able to return to work and engage in other light activities. Vigorous activities, like exercise and lifting heavy objects, must still be avoided for another five weeks, however. Dr. Wilson will arrange multiple follow-up appointments with you throughout your recovery in order to gauge your progress and help you stay comfortable. When you have healed completely, he will let you know when you’re ready to exercise again.

Who Makes an Ideal Candidate for Breast Augmentation?

Women of all ages can have breast augmentation surgery, provided that they’re in good health. Certain health conditions, like diabetes, cardiovascular disease, immune system disorders, and bleeding disorders can make you ineligible for invasive surgery. When you come in for a consultation, Dr. Wilson will review your complete medical history (including any medications and supplements you’re taking) in order to make sure that you can safely have this procedure. Breast augmentation candidates must also be willing to give up smoking (if they smoke) for at least three weeks before and after their operation.

Finally, if you have not had a recent mammogram, you will need to have one before your breast implants are inserted. While breast implants do not increase the risk of breast cancer, they can make unfavorable changes in breast tissue harder to detect. For this reason, it’s important to be examined prior to having breast augmentation surgery.

BREAST AUGMENTATION FAQS

Implant selection is patient specific. There are 4 factors to consider 1. Shape 2. Texture 3. Size 4. Fill. The majority of breast augmentations in the United States are done with smooth, round implants. In most cases Dr. Wilson uses smooth round implants but in rare instances a textured implant may be necessary to provide the ideal patient outcome. The size of an implant and the dimensions (such as high profile) are based on the consultation and desired outcome. Patient’s generally choose silicone gel implants since these have a more natural feel.

Yes. There is typically a 3 cm scar placed under the breast fold that is easily hidden in clothing and bathing suits.

Most patients can breast feed after breast augmentation. There is no known risk to the baby.

Implant shells can rupture. Different manufacturers quote different rates of implant rupture. Modern silicone implants are “cohesive” meaning unlike prior silicone implants these implants do not leak but keep their form. MRI can easily detect the rupture of a silicone implant if this is ever a concern. When saline implants rupture the volume of the implant is lost. The saline is reabsorbed into the body without any consequence.  

Yes! Mammograms are still possible following a breast augmentation. The mammogram technologist will perform special views to visualize the breast tissue.

Currently there is on going investigation to determine the link between implants and a lymphoma known as ALCL. There is a correlation between textured implants and this form of cancer.

Silicone gel implants are the most natural feeling and appearing.

Loss of nipple sensation is a very rare complication of breast augmentation. More often there is an initial hypersensitivity of the nipple that persists for one or two months following the surgery and resolves as the implants settle into place and the skin stretches.

Dr. Wilson does not use drains in his breast augmentations.

All sutures are dissolving and will not need to be removed after your surgery.

This depends on what you do for work. In general, patients with most office type jobs can expect 4 to 7 days of downtime before returning to work. Patients with more physical jobs can expect closer to two weeks.

We recommend that you wait 2 weeks before you start light exercise. Light exercise includes activities that do not use your arms such as treadmill walking, stationary bike, etc. Between two to six weeks you can start increasing your activities and then fully incorporate your arms into your activities.

It is completely normal to have a tight feeling of the breasts following your surgery. It is ok to move and stay active and, in fact, this is good as it prevents blood clots and helps with the healing process. You will not hurt anything if you follow the restrictions on heavy activities.

We recommend you wear a bra following you augmentation to provide support to the implants and the healing tissues. You should not wear an underwire bra until six weeks have passed. This is to protect your incision.

Yes. This operation is performed under general anesthesia and typically takes between ½ to 1 hour.

Given that you will have pain medications and general anesthesia we recommend that you have someone stay with you the first 24 hours. You will also need a ride to and from the procedure center.

Most patients who undergo breast augmentation will at some point need a revision or exchange of their implants. There is no definitive lifetime to any implant; however, as your breast tissue and skin ages changes are often needed to preserve a natural look and position of the implant.
